Much as I understand this is distressing, its not as simple as "removing a name from a database" in large companies and institutions. You may think everything runs from one central location but it probably doesnt, and I expect the details are spread around in lots of different places.

The NHS were trying to centralise their computer system, but currently its £4billion over budget and 7 years late.....And its still not here. So in fact the NHS is probably running on systems that are 15 years old.

yer not alone bernie,no consolation i know, but someone i know very well, who's dad died also in december, gets crap like this frequently.:(

Bernadette, I know how upsetting something like that can be.

It was about 2 years after my husband died that I received a letter from the Solicitor's Office of the DSS. It was addressed to my husband and said, in effect, that they were going to take him to court for non-payment of his National Insurance Contributions. I wrote to them, immediately, pointing out that the reason he had not paid his NIC was because he was dead and that, for good measure, I had been receiving a State Widow's Pension, based on his earnings, and sanctioned by their department, under his NI number since he died.

In my particular Civil Service dept., HMRC, a letter such as the one I received would have been followed up by an abject apology. I received absolutely nothing from the DSS, not even an acknowledgement, but I was too weary to take it any further.

The problem lies, largely, in computerisation of various agencies. Once a button has been pressed, even inadvertantly, it's often impossible to halt the process of issuing a letter/form/statement.

So in this world of high technology where all computers talk I/we should be prepared to accept second best?? I don't think second best is good enough!!!

I agree entirely.
Bring back the personal touch imo.
Sorry to hear what happened,the same thing has happened to me a few times over the years regarding my mum.
It is distressing when it happens and think a massive apology is in order at the very least.

I for one would never dream of claiming compensation, I would be more than happy if I got an apology:(
