I bet you Doc wore a wrist watch, many do and you can't wash your hands properly with a wrist watch or long shirt sleeves on.
The last time I was an in patient, the ward was a shambles Neil, but that wasn't the fault of the staff, who were excellent. They actually did wash down the beds between patients and the my consultant (which wasn't my own) who was in charge of me, did actually take his watch off and put it in his shirt pocket and washed his hands after seeing each patient, unfortunately it wasn't the same in the clinic
